Find Everett in the main room of the Concourse of Souls and offer your assistance.

Any class can complete the quest. The Bloodied Bone reward is restricted to NEC/SHD.

Find the escaped minion

Travel to the Tomb of the Last Wyrmsbane and look near the entrance for the room with upright open tombs; a cracked wall with a hole is just outside.

The escaped minion has a placeholder.

Head to the Tomb of the Last Wyrmsbane and locate an escaped minion. It spawns within a room near the main entrance of the dungeon that contains several upright and open tombs which are embedded in the walls. A good way to know if you are near the right room is to look for a cracked wall with a hole in it. This wall is just outside the room in which the quest NPC can be found in.

When you've located the mob, kill it and loot the Bloodied Human Head which can be found on its remains.
